Medical reports

We provide the medical review for various medical requirements.

You need to make an appointment for many of these medicals certificates and may be given appointments to see both a doctor and nurse.  Please bring the appropriate documentation for your medical.  This will usually be listed on your form or the associated web site.

The fee for medical examinations vary as each medical demands specific information and tests.

We are able to help you with the following medical reports;

  • Government agencies – WINZ, War Pensions…
  • Immigration.
  • Insurance
  • Drivers Licence.
  • Pre employment.
  • ACC.
  • Divers or other recreational medicals.

If you require any medical examination that is not listed above, please call us.

The medical reports (where required), take some time to collate so we may ask you to return to collect the report 1-2 days after your visit,  or we can fax or email it to the appropriate organisation .

Please let the reception staff know you are coming for a medical so we can ensure we have the right staff and appointment time available.  

As with all medicals examinations at Ngongotaha Medical Centre we ask that you pay for the consultation and report prior to our releasing the document.

If we receive a request for a medical report direct from an organisation, we may contact you to confirm you are happy for us to release the requested information.

  • WINZ  (Work and Income)

Please phone to make an appointment and we can let you know the fee for your medical.

Please bring the following with you to your appointment;

  • WINZ form.
  • Photo identification.
  • Payment for your medical.

 

  • Immigration Medicals

We perform the medical examination for a student visa, work permit, visitor’s permit or residency.

It usually takes 2-3 days to complete the forms.  If you need to have blood tests and x-rays you may need to have those and then return to see the doctor.

Please make an appointment with us and tell the receptionist that you require the immigration medical. We do charge a cancellation fee of 50% of the full fee if you cancel within 12 hours or do not attend for your appointment.

 You should bring the following when you come for your medical:

  • Fill in Section A on your form. If you do not have forms, please visit website www.immigration.govt.nz to print out the appropriate form.
  • Attach a passport photo onto the front page of the form.
  • Please bring your passport
  • Please bring your eye glasses if you wear these.
  • You may need to provide a urine sample (if over 5 yrs old)
  • If you would like to bring a friend or relative for support or help with interpretation, please do so.
  • We require payment in full for your medical fees prior to you collecting your completed forms.
  •  Insurance Medicals

Insurance companies may request a medical when you want so take out insurance.  We can perform insurance medicals at Ngongotaha Medical Centre.  Often your insurance company will send us forms or a ‘request for information’  when you have noted Ngongotaha Medical Centre is your designated practice.

As a courtesy, when we receive these requests, we will phone you to ensure you are happy for us to release information. You can confirm acceptance by phone.

If you have a form sent to you for a medical examination, please complete the form as much as possible prior to your appointment as this will save time when you come.  If you have questions we may be able to assist you with these.
